I am not a medical expert but I have read widely on this subject over a long period of time and this chimes with a lot of my thoughts. For example ( as you mention) it would probably be impossible for an 'ordinary' man to put on a dress and wig, walk out the door and walk down the road. He could not face doing that. But then to not only do that but to walk (confidently and shamelessly) into a women's toilet or changing room! Or to go and teach a class of children!
